• When faxing the form in please include, copy of license, copy of voided check, copy of E&O, and Anti-Money Laundering certificate

• To obtain Anti-Money Laundering certificate go to: http://aml.limra.com/LIMRA/NAILBA_Register123.aspx and register as a new user.  In Step 2 select Leaders Partners Inc and in Step 3 select the carriers you wish to obtain appointments with.

Important Information: ING is now requiring you to submit applications before they issue Agent Numbers.  So your first application must be by PAPER APPLICATION.  MAKE SURE TO FAX US your 1st application so we can connect it with your appointment work and get you and AGENT NUMBER.

• On the application you will need to return the following:

1. Pages 4-9 (individual application) signatures from you and the client

2. Page 13, Medical Record Release- signed by customer

3. Pages 18-19, Blood Testing Consent – signed by customer

4. Payment information pages 16 or 17 depending on Credit Card of EFT , customer signature

5. Page 12, Agent Report only filled out by you.  

6. Sign Accelerated Benefit Rider Disclosure if the form is available.  This form is only available in certain states.
